At LOVINA MEDICAL we can perform your immigration physical any day of the week except sundays. Please call and make an appointment for your exam. The physical exam will take about an hour to complete and the result is usually ready in approx three business days.
If English is not your native language, you may wish to bring someone along with you to help answer questions and fill out forms.
The following tests are required <http://www.ins.usdoj.gov/graphics/index.htm> for anyone age 15 or older and will be provided by our office:
Skin test for tuberculosis (PPD);
Blood test to check for AIDS ( HIV);
Blood test for syphilis (RPR)
Certain immunizations are required, DPT, POLIO, HIB, MMR, HEPATITIS, VARICELLA, PNEUMO, INFLUENZA, ETC and if you are not able to produce proof that you have had these, you will need to have them administered as part of your immigration physical exam depending on your age. You will be asked if you have ever had the following illness
Varicella (Chicken Pox).
The following information should be brought with you to your visit:
Your "I-693" form (Medical Examination of Aliens Seeking Adjustment of Status <http://www.ins.usdoj.gov/graphics/formsfee/forms/i-693.htm>), which is the multi-page medical form. If you do not have this form, we may be able to supply you with one at the time of your visit.
Your medical and vaccination records if available.
